REGISTRATION/LIMITED Fully complete the registration form. Incomplete registration forms will be returned for complete information. Placement in Fireground Search: That Others May Live course is not completely reserved until the registration form is properly completed and payment is received. A registration fee of $375.00 for the course must be paid by May 4^th, 2007. All registration payments received after May 4^th, 2007 will be subject to a late processing fee of an additional $50.00. All payments are made payable to RICOFIRERESCUE INC. an applied for charter not for profit organization. Each attendee must be individually registered. Registrations may not be shared among multiple attendees. Individuals and departments not paying with a purchase order (PO) must provide payment using a money order, bank check or cashiers check only. No personnel checks will be accepted. Check payments made through banks other than US banks will need to provide a $25.00 processing fee bringing the check total to $400.00 made payable to RICOFIRERESCUE INC. Individuals or departments paying with a purchase order (PO) for Fireground Search: That Others May Live will be billed $375.00 accordingly and expected to be paid in full within 45 days of the invoice date. Purchase order (PO) registrations regarding cancellations and their payments are not refundable after April 14th, 2007. Purchase order (PO) registrations occurring after May 4th, 2007 will be billed an additional processing fee of $50.00 bringing the total payment to $425.00 and are expected to be paid within 30 days of invoice date. Individuals or departments paying by check for Fireground Search: That Others May Live will receive acknowledgement of payment and confirmation for registration. When paying by check for registration fees before May 4th, 2007 the cost will be $375.00. Individuals or departments paying by check for Fireground Search: That Others May Live after May 4^th, 2007 will be charged an additional $50.00 late processing fee bringing the payment amount to $425.00. Individuals or departments paying by bank check, money order, or cashiers check wishing to cancel prior to April 14^th, 2007 will be refundable less a 3% service fee. Individuals or departments paying by purchase order (PO), money order, bank check or cashiers check wishing to cancel after April 14th, 2007 will not be refundable.
OTHER IMPORTANT INFORMATION The course registration and its size are limited. Hands-on training and evolutions begin promptly at 9:00 a.m. to approximately 4:30 pm each day. Lunch is made available at all training sites consisting of hot and cold foods and a variety of fluids through a commercial vender at a charge of $7.00 per student. It is suggested that some type of drinking bottle or bottled water be brought in by the student although water will be provided at all the sites. Signed liability waivers will be required of every student and instructor at every location and facility involved in hands-on training. Full turnout gear along with an SCBA and extra air cylinder will be required for each day of hands-on training. All SCBA’s and spare cylinders will be OSHA, NFPA, and NIOSH compliant. All attendees should make sure that there name or department affiliate is on their gear. Each student will receive a 35 page text handout entitled Fireground Search: That Others May Live and an events T-shirt made available in large and extra large sizes. Along with this will be a “Certificate of Attendance” for the course at the conclusion of the program.
WHEN and WHERE TO ARRIVE/CHECK IN REGISTRATION All students will arrive for check in registration on May 14th, 2007 at 0800 hrs at the Elmhurst Fire Department’s Training Facility located at: 910 N. Addison in Elmhurst Illinois.

All students will have with them full turnout gear, SCBA and an extra cylinder. Students should also provide proof of payment and registration receipt from RICOFIRERESCUE. Registrations occurring after May 4th, 2007 may have this information of proof of payment at the site desk. Directly after registration and orientation students will be dispersed to there hands-on site locations for the start of immediate hands on training. Have A Great Week!

PAYMENT All reservations must be paid in full prior to course admittance. All checks & PO’s are payable to RICOFIRERESCUE INC. Purchase orders (PO) enclosed by mail or sent by fax will require an authorized signature. Expect a billed invoice within 14 days. All department purchase orders (PO) are to paid in full within 45 days of invoice date except those department purchase orders (PO) received after May 4th, 2007 in which will be paid in full within 30 days of invoice date.

CANCELLATION/WITHDRAWAL POLICY Written cancellations received prior to April 14th, 2007 for all types of payments will be assessed a 3% handling fee with the balance being fully refundable. All other cancellations whether written or verbal after April 14th, 2007 will not be refundable. Please note that all fees and training expenses may be tax deductible; consult your tax adviser.

The author who is also the originator of the hands on course do not assume any profits, royalties, from the course, its sponsors or contributing departments.
