for profit organization.
Each attendee must be
individually registered.
Registrations may not be
shared among multiple
attendees. Individuals and
departments not paying with
a purchase order (PO) must
provide payment using a
money order, bank check or
cashiers check only. No
personnel checks will be
accepted. Check payments
made through banks other
than US banks will need to
provide a $25.00 processing
fee bringing the check total
to $150.00 made payable to
RICOFIRERESCUE INC.
Individuals or departments
paying with a purchase order
(PO) for Commanding the
Mayday and Rapid
Intervention Operations will
be billed $125.00
accordingly and expected to
be paid in full within 45
days of the invoice date.
Purchase order (PO)
registrations regarding
cancellations and their
payments are not refundable
after May 3rd,
2007. Purchase order (PO)
registrations occurring
after May 20th ,
2007 will be billed an
additional processing fee of
$25.00 bringing the total
payment to $150.00 and are
expected to be paid within
30 days of invoice date.
Individuals or departments
paying by check for
Commanding the Mayday and
Rapid Intervention
Operations will receive
acknowledgement of payment
and confirmation for
registration. When paying by
check for registration fees
before May 20th,
2007 the cost will be
$125.00. Individuals or
departments paying by check
for Commanding the Mayday
after May 20th,
2007 will be charged an
additional $25.00 late
processing fee bringing the
payment amount to $150.00.
Individuals or departments
paying by bank check, money
order, or cashiers check
wishing to cancel prior to
May 3rd, 2007
will be refundable less a 3%
service fee. Individuals or
departments paying by
purchase order (PO), money
order, bank check or
cashiers check wishing to
cancel after May 3rd,
2007 will not be refundable.

OTHER IMPORTANT INFORMATION
The course registration and
its size are limited.
Hands-on training and
Classroom begin promptly at
8:30 a.m. to approximately
4:30 pm each day.
Lunch will be on an
individual bases. A 1 hour
lunch break will be provide
to allow students to access
several restaurants
available in the immediate
vicinity of the classroom
and hands on facility.
Signed liability waivers
will be required of every
student and instructor at
every location and facility
involved in hands-on
training.
Full turnout gear is not
required to attend
Commanding the Mayday and
Rapid Intervention
Operations. What will be
required of every
participant is a compliant
Firefighters helmet or
equivalent, structural work
gloves and steel toed
footwear.
Each student will receive a
25 page text handout
entitled Commanding the
Mayday and Rapid
Intervention Operations and
an events T-shirt made
available in large and extra
large sizes. Along with this
will be a “Certificate of
Attendance” for the course
at the conclusion of the
program.

WHEN and WHERE TO
ARRIVE/CHECK IN REGISTRATION
All students will arrive for
check in registration on May
31st , 2007 at
0800 hrs at the
Elmhurst Fire Department’s
Training Facility located
at: 910 N. Addison in
Elmhurst Illinois.

Directly after registration
and orientation students
will be dispersed to there
hands-on site locations for
the start of immediate hands
